Client Forms

Every client is required to sign and fill-out four documents (which are included in the New Client Packet below), prior to the first session:

  1. New Client Form
  2. Family Background Information Form
  3. Counseling Services Informational Contract
  4. Professional Disclosure Statement

 

New Client Packet

  • Click here to download the Microsoft Word (.doc) version to edit on your computer and email back to me. You can sign the appropriate pages at the first session.

 Optional Documents
